If you follow the arrow on the sign above backwards, it clearly means that if you happen to be driving a yellow, rhombus-shaped vehicle, you should go around the island. All other vehicles don't need to.
In general, the yellow diamond means "caution". That's probably to indicate that the "swerve, avoid the traffic island for the love of God!" sign above requires additional caution above and beyond the cautionless "Soft Shoulder", "Through Traffic Merge Left", etc signs.
